Visiting Nurse Health System partners with several government agencies and other nonprofit organizations to help provide long-term care services to older adults with chronic diseases and disabilities as well as others with functional impairments.  These critical services help these clients do what they most want to do -  stay in their homes and connected to their families, friends and communities in an affordable, safe way. 

One of these programs is the Community Care Service Program (CCSP), managed by the Georgia Department of Human Resources (DHR) and administered locally by the Atlanta Regional Commission. Our team of Registered Nurses and Social Workers provide long-term care management, education and clinical oversight for over 1,900 clients at any given time, allowing them to access community-based care as an alternative to nursing home placement.

Visiting Nurse Health System has contracts with Fulton and DeKalb counties for similar programs and is a SOURCE Enhanced Primary Care Management Provider in the ten-county, metro Atlanta area.  This program serves frail elders and adults with disabilities and is designed to delay or prevent the need for hospital and nursing home admissions by integrating primary medical care with home care services.

Health related and supportive services that are coordinated by our professional teams in all four of our community-based programs include assistance with meal preparation, hygiene and nutrition, in-home respite care, light housekeeping, errands, adult day care, skilled home health care, outpatient rehabilitation services, in-home emergency response services, and hospice care.

Additionally, we educate and inform hospital discharge planners, health care providers, and primary care physicians to help them determine which patients are most at risk.
