Patient Connections

Visiting Nurse | Hospice Atlanta is positioned to thrive in response to the community’s growing need for our homecare and hospice services.  Our patient outcomes are among the best in Georgia, our hospice census has more than doubled over the past several years, relationships with key business partners are growing, a talented leadership team is in place and our loyal donor base is expanding.

Patient Connections is our three-year plan to further infuse technology into care delivery and is critical for our continued success.  Launched in January 2007, Patient Connections is a $3 million immediate capital need designed to enhance the house call and allow us to continue setting the standard for quality and access as a community-based, nonprofit home health and hospice provider competing with for-profit, national chains.

  • improved point of care technology

When fully implemented, Patient Connections will improve patient outcomes, extend the caring reach of clinicians, enhance communication among care team members and raise patient and family satisfaction.  We are so grateful for the community’s outpouring of support for this critical initiative!
